Summary:

Accidents don’t just happen. They are caused by something, and usually several things. An investigation should determine these causes, but the cause of an accident is not the same as the cause of an injury. Injuries can be caused by overexertion, falls, or hitting objects. However, the factors that caused the accident are more complex. For example, a wet floor may cause a fall, but determining why the floor was wet and why the hazard wasn’t addressed will get to the root causes.

The investigation of an incident or near-miss should not end with the discovery of an obvious cause; it should continue until all underlying factors are identified. If these root causes are not identified and addressed, they will continue to lead to incidents. Effectively addressing root causes should help prevent recurrences, which in turn will lead to fewer injuries.
