Ohio Turnpike closed for five hours after fatal accident involving three vehicles

WASHINGTON TOWNSHIP – The Ohio State Highway Patrol’s Milan Post is investigating a fatal three-vehicle crash that killed a Chardon woman Monday.

The accident occurred at 9:50 a.m. on Monday, July 22, on the Ohio Turnpike, east of the Ohio 51 intersection in Washington Township, Sandusky County.

Samantha Smith, 35, of Chardon, died at the scene. She was transported by the Sandusky County Coroner to Mercy Health – St. Vincent Medical Center in Toledo.

The preliminary investigation revealed that a Kenworth T880 tractor-trailer driven by Timothy D. Moulton, 58, of Toledo, was traveling west on the Turnpike when it left the right side of the road and struck a concrete safety barrier.
